1. Assumption of Risk
I, the undersigned participant, knowingly and voluntarily agree to participate in a sound bath class (the “Activity”). I understand that sound baths are a complementary, alternative, and holistic wellness practice designed for relaxation and stress reduction.
I am aware that while this practice is generally considered safe, participation involves certain inherent risks. These risks may include, but are not limited to, potential emotional releases, temporary lightheadedness, or slight physical discomfort during deep relaxation. I acknowledge that my participation is purely voluntary and I assume full responsibility for any risks of injury or discomfort that may result from my participation in the Activity.
2. Health and Medical Disclaimer
I represent and warrant that I am in good physical and mental health and do not have any medical conditions that would prevent me from safely participating in the Activity. I understand that the Activity is not a substitute for medical advice or treatment. If I have a serious medical condition (such as epilepsy, severe heart condition, or am currently pregnant), I have consulted with a qualified healthcare professional before participating and have received their explicit approval.
I will inform the instructor immediately if I experience any pain, discomfort, or concerning symptoms during the session and understand I am free to leave the session at any time.
3. Release of Liability
In consideration for being permitted to participate in the Activity, I hereby release, waive, discharge, and covenant not to sue the instructor, Kirsteen Jones, its owners, employees, agents, or volunteers (collectively, the “Releasees”) from any and all claims, demands, losses, or damages I may have or sustain as a result of my participation in the Activity.
This waiver includes claims based on the negligence of the Releasees. I have had the opportunity to ask questions regarding the Activity and my participation.
4. Acknowledgment of Understanding
I have read this Waiver and Release of Liability form, fully understand its terms, and I understand that I am giving up substantial rights, including my right to sue, by purchasing I am doing so freely and voluntarily.
